If you lose your debit or credit card, or it is stolen, you can use this form to block and deactivate the card.
- Select Self Service Options in the left hand menu.
- Then select Report a Lost/Stolen Card.
- Select which card you wish to block.
- Then select Submit
- Select the reason which you are blocking the card. Either Fraudulent Transactions on the Account, Card has been Stolen, At my Request, The Card Holder is Deceased, Received Compromise Notification and Card is Lost.
- Then select Submit.
- A confirmation page will then appear, if you select Confirm the card you have selected will be blocked.
- If you select Cancel you will receive a message of the cancellation.
- When you are finished select Submit.
